We have an extremely exciting opportunity to join our ICB as Population Health Management Director. Our ICB was formed with four overarching aims: to improve outcomes in population health and healthcare; to tackle inequalities in outcomes, experience and access; to enhance productivity and value for money; and to help the NHS support broader social and economic development. We have an aspiration to enhance the health and care outcomes for the populations we serve by working more effectively together and harnessing the skills and expertise across our system.
This new and pivotal role will report to our Deputy Chief Medical Officer and support the work of the medical directorate in delivering high quality, safe and effective care across care pathways within Joined Up Care Derbyshire whilst delivering Population Health Management, prevention and the effective reduction in Health Inequalities. The successful candidate will lead our Population Health Management function in helping drive an evidence informed approach to care that will transform health, care and well-being outcomes for the citizens of Derbyshire.
